Welcome to  Tucson Neuroscience Center/Scott C. Forrer, M.D.

This office specializes in neurological adult patient care. Dr. Forrer graduated from the Michigan State University College of Medicine in 1986, completed his Residency at the University of Arizona in 1990. He is board certified and has been in practice in Arizona for 18 years.

We are located in the Rillito Corporate Crossings Center at 4015 E. Paradise Falls Dr., Ste. 132. Please refer to the map and information located under Office Info Tab.  Our office hours are 8:00am to 5:00pm Monday through Friday. If you have any questions regarding an appointment please call our office at 520.721.1000.

    To provide you with the most complete and efficient service please, "HAND CARRY" the following:

• Medical records (medication list, doctors notes & labs result for up to 1 year)

• • CT scans and MRI's (reports only, no films)

  • Insurance card, referral and/or Doctor's orders (must be brought in order to be seen)

  • Personal injury cases "MUST BRING" accident report and Attorney's name and complete address, name of person responsible for accident, name and address of insurance company responsible for payment

• Worker's compensation cases "MUST BRING" insurance name , address, phone #, claim #, and date of injury.
